The Psychiatric World of Ward 21 PN Hospital and other matters.

Update 29/8/17:  Ward 21 is ticking along nicely these days.  Seems staff have implemented changes and clients are reaping the benefits.  My comments about Dr Starr have been rescinded as he has taken extended leave and a nice psychiatrist or two are in community MH and on the Ward.  Chur!!



I don't bear grudges or animosity to any one person or any one thing. But I do remember those that gave me a leg up when I needed it, and also those that were next to useless (though that's being unfair on Useless). As an eleven year veteran of the mental health system personally (and aware of psychiatric issues through family acquaintances) I have come to inform myself on who is good and who is not. Eventually this tale will spin to Ward 21 Palmerston North Hospital, but for now, I need to cover previous hospitals and staff to build the picture.

My journey started with the CATT Team in Auckland's North Shore, who were called by my wife as I was unresponsive mentally and physically in not much better shape. They admitted me to North Shore Hospitals Mental Health unit called Taharoto and there I began to recover. I was seen by Dr Fraser, a rather serious and thoughtful character, my first psychiatrist. At this stage I gave him my full vote of confidence, but that was the last time I was seen by him. I was passed on to an intern (she was very good - for me) and my recovery continued apace. However after being diagnosed and discharged I was soon back in Taharoto for another stay, and upon recovery I was discharged. During my two stays in this hospital I was treated very well, by all staff, though I did find Dr Fraser kind of aloof. I had a short stay in the community, seen by a very nice psychiatrist by the name of Dr Cummings.

I shifted down to Foxton not long after and came under the auspices of Horowhenua Health and more importantly Dr Jenny Smith. I found her to be a very competent psychiatrist and I enjoyed our appointments. Alas I had to move to Palmerston North.

I was in Palmy for about 16 months before I had my next brush with Mental Health. I was hospitalised again and this time I ended up in HNU (High Needs Unit or Acute Ward). This was the very first time I had dealings with Dr Starr, the resident psychiatrist. I soon learned that I couldn't trust this man, and when we had our (limited) consultations he struck me as an arrogant prick and one not there for greater good. I learned very quickly to loathe the man, and I soon didn't want anything to do with him. Now there are not very many people in this world I have issues with, so Dr Starr is the exception rather than the rule. In fact Ward 21 struck me as a very sterile and uncaring place. Many times I asked why it didn't have a Maori name and why Manawaroa wasn't carried across (the old mental health facility at Palmy Hospital) Most of the staff didn't know or didn't care. And therein lies my issue with Ward 21 (the nursing station is referred to as Enterprises Bridge) the place is just not a warm place to be, and when you have idiots like Dr Starr (formerly Dr Nikolai [sp]) at the helm it's no wonder the staff don't give a monkey's arsehole.

So what is Dr Starr. He is the resident head psychiatrist for the main psychiatric ward as well as having clients on the community mental health register. He isn't highly respected, by either clients in his care, or even staff on the main ward. I have many anecdotal tales of how poorly he is regarded from both sectors, and some of the tales beggar belief. I have had two fellow clients tell me that he (Dr Starr) turned up for their appointments reeking of alcohol. Is this unusual? Well I have been associated with mental health services for seven years and in that time this is the only case that I am aware that a mental health worker presented in such a state. Is this unprofessional behaviour?

I know psychiatry is a mind numbing business and there are cases where psychiatric nurses and doctors have had to use their own services for their care, so I guess it is a very tough business. But surely it is like all hospital care, the staff need to present daily in a unified approach to care for their clients/patients. So Dr Starr, you lose even more kudos points for presenting to a client or two in a hungover state AND offered them your wisdom (which I gather not many people listen to anyway).

My opinion of Starr amounts to these two observations. The second time I was on the ward I was treated like a second class citizen by the good doctor and as such had cause to ask for a second opinion and be serviced by another Psyche doctor. I never received either, so I blamed (rightfully or wrongly) Dr Starr and from there on in refused to listen to him. I recall one day being told by ward staff to see Doctor Starr for my weekly appointment and jokingly told that staff member that if I had to talk to Starr I wouldn't be held responsible for my actions. Now I'm not a violent person, far from it, but they thought I was and so when Starr came on the ward, I was locked away in HNU until he had finished. So you are gathering I have very little faith in the man, further exacerbated by others accounts. Funny thing, when I had to see Starr on discharge, he rambled on for about 45 minutes and at the end asked me if I understood what he had said. Yup I didn't hear the question either until he tapped my hand and got my attention. I just wanted out of there.

In summation, Mental Health services in Palmerston North have this man as it's senior clinician and a lot of people that work for and with him want him gone. So why is he still there if there is a huge vote of no confidence in him, from staff and clients? I guess that's solely down to "The Old Boy Network" at the hospital.

Oh and one last thing. He claims he is the foremost specialist on Bipolar Disorder in this country and yet his name doesn't come up on Google searches. I guess when he masturbates he does at good job of it, and ambidextrously.

  1. Interesting, you just lucky youre not in Wellington mental health system, a white male has zero chance of getting proper care or put in a ward, you either get put in the police cells, or released straight away by CATT, thats if they even bother to show up in the first place.

  2. Mid central mental health system is utter crap these days. I was under them as a teen and they were great. Now They just don't care. Actually I am hopefully on the tail end of an complaint about them about the treatment. And I am still being left to deal with depression on my own (GP doesn't want to know as well).

  3. May of us have had 'experiences' with Dr. Starr. For me, being overcome his large amount of Cologne he tries unsuccessfully to hide the smell of alcohol. Also being told, "you're not allowed to be depressed, you have children and a husband who need you to look after them"... etc.

    I do think it's much harder to fire someone than hire them..... though I know that man must of clocked up a few written warnings by now.

  4. I guess if you could pick your own Psychiatrist the way people get to pick their GP's or therapists then Dr. Starr would be out of a job while some of the others.... Dr. Varghese & Dr. Greennblatt would have more work than they could handle (well they probably already do). Shame though Dr. Varghese not longer works in General Adult Mental Health and Dr Greenblatt refuses to come to Palmy to see even his own clients in the Ward. Dr. Prince (who also works for ACC in the Insensitive Claims Unit) has now shifted to Auckland but she was also a good doctor (yes, even though ACC & good doctors are oxymorons) LOL.

    What I hate MOST about Ward 21 is the Nursing Station and how the Charge Nurse gets student nurses to watch the dramas unfold like a circus sideshow in HNU. It is soooooo humiliating and what does it teach students REALLY? Not like they are even their patients that they are following through to see it in perspective. Plus HNU represents how much of the total population of people with an experience of mental illness??? It does that students far MORE HARM than good. Stupid Charge Nurse they have there presently doesn't get it AT ALL!!

    Oh and LOTS of real lazy staff there too that gossip in the nurses station thinking we (the patients) can't hear what they are saying. How about spending that time talking or doing stuff with your patients? Oh an exception to that rule...... Howard Tripp..... that nurse ALWAYS spends time with patients and is very easy to talk too and very calm.

As anon just stated, I was about to say the same thing ... becareful you could be sued :) It seems Wellington CATT team are just as useless, they let my mentally unwell husband out saying he was normal ... safe to be on his own, I am now widowed, my children fatherless as he sucessfully committed suicide - I guess he is in no harm of himself now, shame he had to leave this earth and his family too achieve it.

  8. Gee sorry to hear of your loss. Maybe this blog could lead to a shake up of mental health services. I know we are working hard to teach the general public and businesses/organisations that having a mental illness isn't an issue, but maybe we need to actually put money into over stretched Mental Health wards and CATT teams and get the best staff for both and train them accordingly and regularly on how to best "care for" those in distress.

  9. I think they treat ya like crap so you don't come back! Mental Health Care is quite SHIT here and if it was actually consumer driven like the Mental Health Blueprint says then it would actually be something for other countries to aspire to and 100 times better than it currently is. But there is NO WAY in hell doctors will let that POWER go. Afterall, why do we need Psychiatrists to be the lead and responsible Clinicians?... Why not Psychologists for people with Personality or Trauma issues with a referral to a Psychiatrist if needed like they currently do with Psychologists? The reason is simple..... doctors are part of the 'old boys network' and they are (or at least were) meant to be all knowing and beyond reproach. Well, none of us are that navie these days.

  10. At least in Ward 21 you get your own room. I hated having to share in Manawaroa with people who were manic or who snored all night! I use to get heaps of stuff stolen in Manawaroa.

    I do hate how ya don't get to pick your doctor though or how you end up having heaps of different doctors over the course of your care. Always feel like I'm repeating myself. One doctor said to me, "I spent hours reading all your notes and to be honest I was a bit apprehensive about meeting you. But you are nothing like your notes portray you".

    I remember thinking..... bloody hell what do they write it there that scares the crap out of people reading the notes. Guess that's the issue when only the BAD or UNWELL stuff gets documented. It's not a true or holistic overview at all :(

  11. Yes I agree! I had Starr as my psych doc in the community and whenever I ended up in ward 21. He's a terrible doctor, and I also hear that the staff dont hold him in high regard. I think he needs to go!!

  12. Its only since Dr Prince left (end of 2010) that they reorganised Ward 21, because they were short of psychiatrists. Before that if you were placed in the ward you were seen by your own community psychiatrist - which was MUCH better, because it was with someone who at least had some knowledge of you as a person and on your history. Dr Nikoloff/Nickalei/Starr is scarily incompetent as far as I am concerned. When I was taking clients to him when I was their support person, he was equally as incompetent. I was put on the ward earlier this year by Dr Ischei (a very good caring psychiatrist) who knows me and felt it was the best place for me to be - to only find that the ward had been reorganised and now has a Ward Psychiatrist, and that Ward Psychiatrist was Dr Starr. Well he, who doesnt know me at all, decided I didnt need to be there. My keyworker told me that Dr Ischei had some 'very strong words' with Dr Starr about it, because it was a horrible position to be put in for me. There was also a new psychiatrist registrar there from Europe who was absolutely irresponsible. But then, I also think registrar Gary Crosby could do with a lot of improving too.

    I agree with your overall view of the ward staff - disinterested, cold, lazy, and not wanting to get involved. This is in contrast to Taharoto where I felt cared for, the nurses were genuine and and empathetic, and just a whole completely different ethos. Taharoto saved my life.

    A couple of the nurses in ward 21 actually do their job, but most couldnt care. Their is one nurse that whenever she talks to anyone its always about 'ruminating' - its gets old real quick!

    I think Ward 21 and its staff need a really good shake up.

  13. this is all very interesting because i too have experienced ward 21, herr starr (as we called him), and many of the experiences others have written about add up to my experience of it too.
    i also know a patient that attacked herr starr, leaving him considerably shaken. believe me, that patient gets props. and now starr won't meet him without 2 security guards.
    Starr is also known amongst the staff as an over-medicator. he will tinker with your meds and add to them - an overmedicated patient is a quiet patient. But he has also gone as far as to medicate a voluntary patient with previously non-charted mellaril to keep them compliant (practically comatose)during a meeting that was expected to go the wrong way. try taking that complaint to the dhb! starr 'lost' the patient's med notes for that day just to ensure the issue never made it beyond the ward.
    Doc Ischei is an incredible doctor that has worked very hard to help me and i have heard that he has taken starr to task on a couple of occasions.

    Ward 21 is not the worst psych ward i've experienced - Te Awhina in Whanganui has that distinction, it is, quite simply,a hole. forensic patients from stanford unit are rehabbed back into the community via te awhina (forensic patients are the criminals with mental health issues who are sent to hospital not jail) and when i was there in 2003, male and female rooms were not segregated. one of the rooms in te awhina sleeps 6 - snorting farting sleeptalking patients. occupational therapy was limited to 2 mornings a week of making candles and shit. when staff levels got low they forced all the voluntary patients (i was sectioned so they could do what they wished) to the high needs unit and locked us in for a shift at a time.
    compared to te awhina, ward 21 is classy, but nothing beats Princess Margaret Hospital in Chch in the early 1990s with their Ward C for eating disorders and postnatal depression. that was kind and friendly and as a long-stay ward, really supported each patient throughout their stay.
    Just to make the collection complete, i must mention Sunnyside in 1992. I can't remember, but it was ward g or h and was very old and decrepit with no doors in the dorms and only curtains in the toilets, instead of doors.

    ward 21 goes through lots of different incarnations depending on the demographics of the patients there. under starr it has been fairly empty with stays kept at under a week. (my stays were 2 weeks and 2 months) i have friends with close contact to the ward so it is interesting to see how the place operates in comparison to how i experienced it. day services has been cut back considerably - it used to be a 25 hour/week programme, nothing like that now. as it has been pointed out, starr is the doctor overseeing each inpatient and despite this attracting criticism, has not appeared to change. it is the most ludicrous approach, resulting in the complete lack of continuity in patient care.

    compare that to the embarrassment of riches one is offered as a cancer patient at midcentral health! i swear i would give up my serious lifelong mental illness for a crack at early detected breast cancer!

  14. after reading all these stories i just dont no what i am going to do. i have been depressed for a long time and have hit an all time low. because of continual suicide thorts and mental issues i no myself i have thort i would try signing myself into ward 21, but after reading this blog im to scared now. i desperatly need help and just dont no what to do

  16. I understand where you are coming from regarding star. I was release from ward 21 not even 2 weeks ago, he told me that Horowheanua team will not release me from there books and guess what I was discharged on Friday. He is utterly useless and I cannot see how he is able to carry now. I know I will end up being admitted back into ward 21 within matter of days if not hours but I am not looking forward to dealing with star if I do. I have heard stories about him turning up drunk as well.

  19. No I have no idea. I do know there have been changes but some staff that have been an issue in the past are still there so I'd not expect too many changes. Generally the CMHT is great, just the ward and psychiatrists may still be an issue.

  21. Sorry I can't do that, I haven't personally needed psychiatric services for over 3 years. Best ring Community Mental Health Team and ask them. Just ring the Hospital and ask for CMHT

  23. I don't know. I was never offered any but my case may have been different. Best ring them and find out that way. I do know Journeys To Wellbeing offer Peer Support services which are hugely beneficial to sufferers.

  24. Sup. Thanks for posting your history. I had a recent situation with Starr. I will say that I was shocked. And I'll leave it at that. I've got your back Gunner Thane. I must say though, I found Ward 21 helpful. However, Dr Starr gave me such a shock I lost faith in humanity. No BS either.

  25. my partner was recently admitted to ward 21, as a voluntary patient due to suicidal thoughts, never having delt with mental health services before myself can anyone please help me as I like to know what rights she has as a patient, & what my rights & ability are (ie can I take her out to spend the day with family) any thoughts or help would be greatly welcomed

  26. Dave the Code of Rights as laid out by the Human Rights Commission are on display in the ward. If you need further clarification ask staff or the consumer advocate to explain them to you. If she is voluntary and not in HNU then she should be able to get day leave but ask staff once again. May only be a few hours though. Old story with Ward 21 if you don't ask you won't get. Good luck and sorry to hear your partner is in the state she is. Here's for a good recovery for you both :) Thane

  29. Ward 21 is an utter mess. Currently trying to cover up 2 suicides since Easter this year. 1 which happened in hnu and the other on the open side of the ward.

  30. Has anyone themselves or someone in their family had anything to do with Ward 21 since Sept 2013 or in 2014 at all? Would love to hear stories of how it has been or what's happened? Hard to get information of what is really happening there and it can't carry on the way it has been the last 10 months or so. Please spread the word and get people to tell their stories here (or somewhere public). It's the only way we can get any change and make people stop covering up.

    4. My good friend killed himself on Ward 21 grounds last year. He had voluntarily admitted himself for low mood and suicidal thoughts. With only a weekend left before the end of his 3 week stay he bought a knife from the dairy across the road. They didn't find him for 5 days and he was only a few meters from the front entrance. His mother parked in front of the tiny garden where he lay every one of those 5 days, looking for him. The duty nurse didn't report him absent from a 30 minute walk until it had been an hour. The Dr pushed the "missing person of concern" procedures 2 hours more to encourage him to get more leave. He was probably already gone before they even thought to follow procedure. This was a couple weeks after the girl who wandered off the ward to committed suicide when she couldn't smoke on hospital grounds. And a month or so before the next suicide in relation to ward 21. So what's changing there?

    5. So very sorry for your loss and family members sorrow. I want to see things change which I have been actively proposing for a long time. Suicide isn't the domain of the Mentally Ill but has a large part to play. Nothing short of a Commission of Inquiry, more qualified staff and better forms of care and money sunk into services. As I said my heart goes out.

  31. I just read read an article on www.stuff.co.nz about a young male who sadly commited suicide this week in ward 21 in Palmerston Norths mental health unit. Very sad. I have bad depression, anxiety and ocd, and have seen lots of doctors, psychiatrists and psychologists over the years here in Tauranga, and although I have met a couple of lovely ones, the rest have been absolutely horrible to me and treated me like shit! So much so that I walked out feeling much, much worse than when I walked in to their offices! I seriously dont ever want to go see a doctor again, i've had enough bad ones, and I would rather suffer than walk out feeling like I wanna kill the over paid bastards.. Sorry for my rant, but I feel all your guys pain. I know what its like being treated with disrespect by the very people who are paid to help. It's not good enough. And it has to stop. Take care all.

  34. I was admitted 4 2weeks in ward 21 my medication had given me hives. But i didnt wot they were so i went 2 two doctorz an 5 nurses to complain about my hives. I gt told by one nurse to rub moistourisers on it an it would go away.

  35. I had been totally ignored for 3days from 5nurses an 2doctorz. After all of that i went to see the doctor about my hives. And they said i dont knw wot it is? If u doctors an nurses stopped being so 2faced. Maybe the patients might be able to get better. Instead of gettng better i gt worse in ward 21. Becus the nurses an docs were part of the reason that i had a melt dwn. After that they have a cheek to say. R u ok? What hospital? Fucken 2faced wankers!

I've had experience in two lovely institutions onw princess Margret hospital in christchrch where the eating disorders and post natal depressive share the same wing, I had people trying to teach me how to purge to help loose my pregnancy weight and they would tell me the exact fat and calorie content of every item I went to eat.....that was nothing compared to the bracelet scars and nearly broken arm I received from the police on a forced admission, calmly dismissed as due to my mental state. No doctor examination until all my brusies were gone and the scabs on my wrists healed.....funny that. Then they forced me to back date it to the day I went in....apparently I was delusional about dates unless I agreed. Freaks covering their own asses. Hilmorton (formally sunnyside) stay nearly 12 years later.....not much had changed people with eating disorders trying to get me to join in their games with the nurses swapping foods and change plates, a poor girl with epilepsy and no medication for it as she was delusional....wasn't until three seizures later they finally got her the right meds, climbing the ten foot high fence with the internal overhang gave some relief from the mindnumbing boredom of the lockdown ward, unfortunately also incurring a padded room stay and lots of tranqs to the bum....but hey you only live once......needless to say avoid self admission where possible and forced admission even more, heaps of great community based treatment without being locked up behind the secure doors. Some of the nurses were real freaks trying to make there lives interesting by causing issues for patients using the notes to try to heighten fears.....if your ever committed never act out in anger just smile a fake smile and talk to the wall behind them.....then you will get left in peace to process your thoughts

  37. Last July I was hospitalised for 3 months.Soo-oooo good to see very positive and heartwarming care on the ward, and positive psychology services. I felt safe and that was very important. Later after discharge I fell under Dr Starr's care in Community Mental Health. My previously recorded sessions were happily improved. Affable, humourous, and willing to discuss matters surrounding my assessment and medication. I'd have to say I have changed my mind and opinion.

  38. Wow.... things have changed considerably. I was in the old Manawaroa. It smelt of stale smoke, toe jams and BO. Many people there were not very well kept or hygenic and many things happened during my 9 month on and off stay. A lady was raped, a room set on fire a deviant was nearly sitting on me playing with his genitals and it took forever to get them to deal with him. It was really the hygeine and smell I remember. The Psych was Dr Vjay and medication was pushed into you willy nilly. Yuck yuck. Oh and absolutely nothing to do and no cellphones in those days. Very bad memories.

  40. Sory iam after information about a Donna smythe ward 21 from about 1983 to early 90s anorexia treated like shit please her brother Donna lost her battle 2014 dew to a fuck up by nelson mental heath would like to know anyone with information back then please

    1. Firstly my thoughts go out to you and whanau and friends. It grieves me to see people suffer. Not sure on exact dates (very hard to find in Google). Bad news (sorry) but up until about 2001/2 the Mental Health unit at PN Hospital was located at Manawaroa. And about that same time Lake Alice was shut down. Fair to say there was a little turmoil surrounding both units closing. You sisters file would I say be in archives now and memory says that's a hard task to get them (when I requested my fathers file was informed past the limitation of 7 years, push hard for information. Once again sorry for your loss. She may be a memory but a worthy one for you.

    1. Today, I had my first appt with Dr Starr, who has replaced another psychiatrist who was amazing, but sadly transferred to another district. It was not a pleasant or helpful encounter and I was feeling very emotional and vulnerable. He was going on about various things that was not helpful to me! He very quickly informed me after 10 mins into the session that he needed to change all my meds?? There was no duty of care from my experience and the whole process distressed me enough, that I walked out of the session half way through and sat in my car sobbing. My key worker came after me, but I was too upset to talk. All I wanted to do was return to my safe place (hm). I will not be seeing Dr Starr again, if I hv anything to do with it.

  43. I transferred away from Dr Starr over a year ago and now he's back and I've been transferred back to him. The hell with that, I'm coming off my meds and getting the hell out of Dodge

  46. Rachel (from Horowhenua) I tell you Thane, all we get in our health clinic are old doctors brought out of retirement I think. I have a different one every time I go there pretty much. I thought I had latched on to a permanent one for a while, be he overworked and burned himself out. Today I saw another new old fart, who stank of cigarettes and he just blew me off like I was too much trouble. My health is not good and I need a doctor who cares enough about me to at least to keep on with the tests to see what is wrong with me, but no, this new one is as useless as tits on a bull. All he is good for is handing out pills, when I need answers and help.
    I will tell you what else is new. On the form we take into the doctor from the receptionist, there is a new piece of info that has been added. All the things they use in the doctors surgery that they use on the patients, is now to be charged for. So if you go in there for a consult, don't expect it to be the same charges as usual, cos if you have anything done, bandaging, injections, test sticks for your widdle, cotton buds, sugar tests, you name it, you gotta pay for it. Sop your usual bill, might end up costing you a lot more, so go with extra money in your wallet. Good old Palmy hospital board who runs our clinic.

  48. We lost Dr Prince to ACC, German lady lasted about 6 months, and Dr Seale has gone now too with Starr asked back I suspect to fill shortages. I'll say what I have been saying for years, Mid Central Health is in crisis and no one is caring. It is purely indicative of the national status of Mental Health.

    1. Rachel from Horowhenua It really isn't good enough is it. People are probably already dying from lack of medical care. I am wondering what I will face going in for my op soon. Daughter was thrown out the next day after a double hernia op, with MRSA and a build up of fluid in her tummy. If it wasn't for the district nurses looking after her, I don't know what would have happened. She went back to hospital for a check up and the surgeon and nurses wouldn't even touch her. She had to remove and change the dressing herself while they stood meters away from her. I am praying hard for a change of gummint so we can get rid of that Coleman.

I was a patient in 21 between the years 1970 and 1985 I was in Manawaroa off and on and was given Mason Dury as my doctor I never liked him and then when he started not turning up for my appointments because he was out at Massey a lot I asked for another doctor He asked me why and when I told him he was never there to talk to and my short appointments were always being cancelled he got stroppy and walked out He never spoke to me again but I ended up with a fantastic Dr Maureen Bell She was great I trusted her with my life and she got me going and eventually discharged I suffered from depression over the years I was diagnosed according to others every thing they had a name for I was told I would need continuous treatment outside and in treatment My cocktail of pills was amazing there's not a pill I wasn't put on I used to one part take 9 tablets at night plus they threw sleeping tablet in as well plus ones during the day if one was supposed to feel better it didn't work I felt like a zombie couldn't even stand up properly I eventually got to the stage where this wasn't working for me so I lied my way out of the place went home was given a whole lot of meds I don't condone anyone to do this but what I did was flush all my meds down the toilet and fixed myself by getting a cleaning job and working 6 days a week my depression disappeared never went backwards I put the getting better to Dr Bell She was tough but I loved her She wasn't very popular as her ideas of treatment didn't conform with the others and I heard her arguing often with the other Doctors it worked for me though I did and still do have some pet hates from those times being locked up can't stand any type of board games we were told we had to play them at night didn't have a choice and the other one is I hate being in crowds this is from having people around all the time and never allowing to be alone there was always people around no space of your own Today I live alone and love it I have space and the key to the front door Maureen Bell was my saviour she got me through the bad times and I became a SURVIVOR one of the most important ladies in my life as I'm still here today the best thing I did was speak up and ask for another doctor my future would gave been so different if I hadn't Thank you Maureen Bell
